I can't think clearly. I have this horrible tendency to listen incredibly intently, but then forget names and subjects of what was discussed. My reading comprehension is horribly low as well. Its horribly annoying and my mind is a cloudy mess! Any clue what this could be?

Might be some deficiency, in spite of your healthy diet. Different people have different metabolisms and needs, some need more of certain vitamins or minerals, others less.
Perhaps your diet is too "healthy", i.e. you getting too little fat to soak up the fat-soluble vitamins, or too little salt, etc.
Question: Do you remember visual information (images, symbols) better than words (whether they're written or spoken)? This does matter and I would like an answer.
Regardless of which is the case, you should consider picking up or downloading some books about memorization techniques. They do quite help.
Maybe it's because you lack interest. When I read about something I'm not interested, I forget it real quick. Same with conversations. Even if I'm listening and responding, if it was something that doesn't hold my interest, I forget the conversation even happened.
