I have been having a very hard time recently being able to keep myself focused at work. It has gotten to the point where when I am in my office, I will shut my door, turn off the lights, and sit in my office all day terrified that I won't be able to do my job. And no smart comments saying, "Well, if you're in your office with the lights off, of course you can't get your work done."
I am so busy at work that I am completely overwhelmed, I start doubting myself, my abilities, and end up frozen in fear that I can't do my job.
I am an IT manager for Television station, but the only thing I manage are the assets/systems. I don't have a staff, and I am supporting about 250 systems. I need to find a way to be able to focus and organize myself to be able to take care of the work I need to do.
I don't know what to do.

Have you tried to apply some systematic methodology to your work patterns? Reading about Getting Things Done (GTD) might be a good place to start: http://en.wikipedia.org/wiki/Getting_Things_Done
You seem to lack organization and have reached panic stage. There are solutions to your problems, you just need to try them.
Well, how old are you? This could be a mid-life crisis you know? In which case I'd suggest you to live it to it's fullest and buy the car, house, boat, you always wanted. Do the trip you always wanted!
If you're not feeling motivated and if you can't focus... It is just because it doesn't interest you anymore, so... I think you should first try to understand if there is a reason for you not to care anymore about your job... If you can't find a reason just take a few days off and do whatever.
That is... if you can, or if you know you'll be able to get another job that'll satisfy your needs.
Which reminds me... Why don't you just change your job? Don't force yourself, try new things out.

"Watami Co., Ltd.," formerly called as "Watami Food Service Co., Ltd.," killed a young Japanese lady by violation of Labor Standards Act.
(ワタミ株式会社、かつてワタミフードサービス株式会社と呼ばれていた会社は、労働基準法違反で若い日本人女性を一人殺害しました。)
Ms. Mina MORI, who belonged to Watami, killed herself at the age of 26 in 2008, due to mental instability by excessive and illegal hard work.
The bereaved family claimed workers' compensation around four years ago. As a result, the examiner of Workers' Accident Compensation
Insurance officialy affirmed the petition, and the determination letter dated on Feb.14, 2012 says; "She could not afford holidays at all
because her overtime work is more than approx. 140 hours per month. We suppose that the main cause of her suicide would overtax by
excessive hard work."
A store manager and a careworker, both of who belonged to Watami, witness similar hard work and depose with evidences that Watami
violate the Labor Standards Act, especially constantly unpaid overtime work. If any of them were true, Watami commit organized crime.
